*VOLUNTEER REQUIREMENT*
For student safety all volunteers in any capacity at school events must have a completed and approved Volunteer Application Form on file at the Polaris office and you must have a completed and approved SD57 specific Criminal Record Check report on file at the Polaris office. Criminal record checks must be renewed every 3 years to be valid.
ALL volunteers are required to have this paperwork filed prior to the event they are helping with. If you need assistance with these specific forms please see the Polaris Secretary. Thank you!
